MSHP is a clinically integrated network comprised of Mount Sinai's full-time faculty physicians, associated community physicians, and eight hospitals spanning Manhattan, Brooklyn, Queens, and Long Island. Our doctors work closely together and hold each other accountable to deliver high-quality health care to people that is rooted in the best practices of current evidence-based medicine.
As a clinically integrated network, we don't just take the pulse of our patients—we take ours too. Brent Estes is the Senior Vice President and Chief Managed Care Officer of the Mount Sinai Health System. In this role, Brent provides overall executive oversight for administrative, contracting, and financial performance of value-based, risk, and fee-for-service contracts. He works with the Health System finance team to drive the system’s overall revenue strategy, with Population Health leadership to shape the value-based agreement strategy and drive performance.
Most recently SVP, Business Development at Advocate Aurora Health, Brent led development of an integration plan to drive system-wide care coordination and improve operating margin, as well as sponsored a joint venture with a national health insurer to deliver a high-performance network in multiple markets. Prior to that, Brent served as SVP of Business and Network Development for Rush University Medical Center. At Rush he designed and executed a multi-year payer contracting strategy, and transformed a physician-hospital organization operating model into one of the most mature clinically integrated networks in the nation.
Brent also has experience teaching at Rush University, College of Health Sciences, Department of Health Systems Management in Chicago, where he was a faculty member and instructor as well as a course director.
Brent is a member of a variety of professional organizations. He earned his degree from University of North Texas.

As Senior Vice President and Chief Medical Officer for Population Health at Mount Sinai Health System, Dr. Rahim leads, innovates, and transforms clinical operations to ensure that we deliver high-quality, efficient health care services to the more than 460,000 Mount Sinai patients who are a part of our value-based care and performance-based contracts.
Dr. Rahim leads a team that drives clinical integration and performance excellence in value based care performance at primary care, multispecialty and select specialty practices. Dr. Rahim is focused on clinical integration across programs that reduce avoidable acute care utilization, quality performance, accurate risk adjustment, and physician satisfaction. Dr. Rahim co-leads ambulatory condition management efforts in high cost chronic diseases including Heart Failure, Diabetes, COPD and HTN. He also leads and closely collaborates with system stakeholders for CIN recruitment, network curation, and key account development activities.
Dr. Rahim has over 15 years of healthcare industry leadership experience at innovative companies including as Vice President of Quality Improvement for Lumeris, a population health and analytics company; a Group Vice President of Quality Improvement and Innovation at Healthgrades; and a Director at Sg2, a health care intelligence, analytics, and services company for many hospitals and health systems throughout the country.
Dr. Rahim has an M.D. from the University of North Carolina, and an M.B.A. from Emory University. He completed his internal medicine residency at Yale University and Northwestern University and is an actively hospitalist at the Mount Sinai Hospital.

Maria Alexander is Vice President for Population Health Operations at Mount Sinai Health System. She provides overall executive oversight for operational, administrative, clinical, and financial areas of population health and clinical integration operations. Ms. Alexander’s goal is to create, maintain, and optimize a world-leading population health infrastructure in support of value-based care principles supporting the more than 460,000 Mount Sinai patients who are a part of our value-based care and performance-based contracts. She also serves as the Chief Operating Officer of Mount Sinai Care, LLC (Mount Sinai’s Accountable Care Organization).
Prior to serving as VP, Ms. Alexander was the Senior Director, Population Health Clinical Operations and Government Channels for Mount Sinai Health Partners, where she managed governance of Mount Sinai Health Partners’ Clinically Integrated Network, oversaw Mount Sinai’s participation in the Medicare Shared Savings Program, and advised on government payer programs and policy.
Prior to joining Mount Sinai in 2018, Ms. Alexander spent six years at the Centers for Medicare and Medicaid Services (CMS), including as a Division Director in the Innovation Center. During her time at CMS, she helped develop the Pioneer ACO Model and the Comprehensive ESRD Care Model, and worked on several initiatives focused on dual-eligible populations.

Scott Edwards, MBA is the Vice President, Value-Based Programs and Finance for Mount Sinai Health System.
Mr. Edwards oversees and drives the development, negotiation, and implementation of the Health System’s risk-based contracts with commercial and governmental payers. He ensures that the expected and actual outcomes of such negotiations are factored into our overall contracting and growth strategies. He also oversees the finance functions of Mount Sinai Health Partners and represents Population Health in Health System finance forums.
Mr. Edwards has extensive leadership expertise within both the provider and payer managed health care industry. He most recently served as Senior Director of Pricing Strategy and Value-Based Performance for Cleveland Clinic. In this role, he had enterprise-wide accountability for reimbursement strategy and performance in value and risk-based contracts.
Prior to Cleveland Clinic, Mr. Edwards held Vice President and Principal advisory roles with both Conifer Health Solutions and Optum, where he led development and implementation of strategies to help provider and payer organizations achieve success in value-based care and population health initiatives. Mr. Edwards has also worked at Cigna and UnitedHealthcare, where he held roles in financial, analytical, and total cost of care leadership.
He earned his Masters of Business Administration (Finance) from Millsaps College.

Kwynn Smith is the Vice President for Payer Contracting for Mount Sinai Health Partners for the various forms of payers. Key areas of responsibility are negotiating and managing the fee for service contracts for the Mount Sinai Health System facilities, Narrow Network, Private Label Products, Specialty Bundles, International Payers, Direct contracts and other areas of Payer related contracting. Prior to joining Mount Sinai Health Partners, he was the AVP of Contracting at Montefiore Health System for over 17 years. Kwynn has over 23 years' experience in contract negotiations and management within the health care market ranging from Hospital to Ancillary and Professional contracting experience.

Edward Bailly, FNP-BC, MSHCDL, is the Director of Ambulatory Quality and Patient Engagement at Mount Sinai Health Partners.
Edward drives the implementation and ongoing improvement initiatives that enable Mount Sinai to deliver better value to its patients, its customers, its providers, and its partners. Edward develops strategies for success in quality among our various contracts and oversees the patient engagement team. His team works directly with clinical and operational leadership to identify and track performance on key indicators, analyze performance against national benchmarks, and serves as the primary clinical point of contact for quality initiatives with departmental and divisional chairs, practice medical directors, and payors’ medical directors. Edward has a B.A. in Economics from Hamilton College, a Master of Science in Nursing from Pace University, is a board-certified Family Nurse Practitioner and received a Master of Science in Health Care Delivery Leadership from the Icahn School of Medicine at Mount Sinai.

Dr. Nikita Barai is the Senior Medical Director for Quality at Mount Sinai Health Partners. Dr. Barai and her team collaborate with clinical and operational leadership across the health system to promote better clinical outcomes for patients, while also driving financial performance across Mount Sinai’s value based contracts and quality programs.
Dr. Barai’s perspective on health system improvement is informed by her work as a primary care physician and clinician educator in the Internal Medicine Associates practice, where she previously also served as the Associate Medical Director for Population Health and enhanced clinical operations and team-based care models to improve quality outcomes. Prior to joining Mount Sinai, Dr. Barai was a primary care physician at New York City Health + Hospitals/Kings County.
Dr. Barai completed her residency in Internal Medicine and Pediatrics at Johns Hopkins and is board certified in both specialties. She earned her M.D. at the Indiana University School of Medicine with Alpha Omega Alpha distinction and holds a B.S. in Urban & Regional Studies from Cornell University.
Between her undergraduate degree and medical school, Dr. Barai served as a Bill Emerson National Hunger Fellow with the Congressional Hunger Center and Health Educator in the IMPACT DC Asthma Clinic at the Children’s National Medical Center. These experiences inspired her to attend medical school and continue to ground her belief that health systems can and must transform to better meet the needs of vulnerable patients and ensure equity in health outcomes.

As Vice President of Care Management and Population Health, Maria leads a team of dedicated, mission-driven social workers, nurses, and care coordinators that are aligned to Mount Sinai’s Clinically Integrated Network. The MSHP Care Management Team aims to find and assist patients at high risk for avoidable utilization. Care Managers meet patients in their homes, at their providers’ offices, and telephonically.
In her 15 years at Mount Sinai, Maria has designed and led several initiatives to improve the experience of illness and care for vulnerable and under-served populations. Her team received multiple years of outcomes-based funding from the Centers for Medicare and Medicaid Services to support social work-led hospital transitions of care, and she has co-authored several publications and presented at numerous conferences on this body of work. Maria is a licensed clinical social worker in New York and California. Prior to joining Mount Sinai, she ran bereavement groups for widows and widowers of 9/11 and provided social work services to chronically and terminally ill patients at Kaiser Permanente in San Francisco.

Dr. Anitha Iyer is the Director of Behavioral Health Population Management at Mount Sinai Health Partners, and Associate Professor of Psychiatry at the Icahn School of Medicine at Mount Sinai. As the Director of Behavioral Health Population Management at MSHP, she collaborates with members of the Clinical Operations Leadership Team, providers within the Clinically Integrated Network, and teams within the Department of Psychiatry to develop necessary pathways, workflows, and programming for behavioral health assessment and treatment. Through a systematic, data-driven approach, she aims to promote better clinical outcomes for patients, streamline evaluation and treatment processes, and optimize provider performance.
Prior to joining MSHP, Dr. Iyer was the Chief Clinical Officer, and Vice President of Crisis and Behavioral Health Technologies for Vibrant Emotional Health, a New York City based not-for-profit organization, and served as an advisor to the National Suicide Prevention Lifeline. She is a recognized leader in the innovative delivery of crisis services, frequent speaker at behavioral health and suicide prevention conferences across the nation, and has served as a subject matter expert for media engagement on matters pertaining to mental health and suicide prevention. She also serves on the Board of Directors of Sakhi for South Asian Women, a New York City based organization that supports South Asian survivors of interpersonal and gender-based violence. She is multilingual and fluent in English, Tamil, and Hindi/Urdu. Dr. Iyer, a licensed Clinical Psychologist, earned her PhD in Clinical Psychology from Columbia University-Teachers College, and her BA in Biology from Barnard College.

Esther Moas, DNP, MS, RN, is Vice President of Care Transitions for Mount Sinai Health System and part of the health system’s Executive Nurse Cabinet. In this role, Ms. Moas is responsible for improving quality of care across the care continuum to reduce preventable admission, re-admissions, and total cost of care. Ms. Moas is responsible for the health system’s post-acute care strategy and operations, as well as population health nursing initiatives. She holds an adjunct faculty role at Mount Sinai’s Phillips School of Nursing where she teaches health policy. Ms. Moas has over 10 years of experience in post-acute care operations. Prior to joining Mount Sinai, she was a Corporate Director at ArchCare overseeing strategic clinical initiatives for their skilled nursing facilities, home care division, and health plans. She started her career as a visiting nurse with a passion to maintain patients safely at home in the environment they know and prefer. Ms. Moas holds a Doctor of Nursing Practice degree from Yale University. She received her Bachelor and Master of Science degrees from New York University.

Joseph M Truglio, MD MPH is Assistant Professor of Medicine, Pediatrics and Medical Education at the Icahn School of Medicine at Mount Sinai. He received his BS and MD from Georgetown University in 2001, and 2007. He trained in Internal Medicine and Pediatrics at the Mount Sinai School of Medicine from 2007 to 2011, where he also served as Chief Resident. He completed an MPH in global health at Mount Sinai, during which time he developed educational curricula for mid-level providers in the chars of rural Bangladesh. Since completing his residency he has served as the Course Co-Director for The Art and Science of Medicine - the two-year clinical skills course for first and second year students at the Icahn School of Medicine. He is the founder of the Mount Sinai Pediatric Visiting Doctors Program, which provides comprehensive, multidisciplinary care in the home, clinic and hospital to medically fragile children. Most recently he helped lead the Mount Sinai Pediatric Associates Clinic through PCMH 2011 and 2014 Level 3 certifications. Dr. Truglio is a board-certified specialist in General Internal Medicine and General Pediatrics who sees patients alongside residents in the Internal Medicine Associates, Pediatric Associates and Pediatric Visiting Doctors Practices at Mount Sinai in New York, where he specializes in care of adults and children with complex conditions.
