Joining Our Clinically Integrated Network:

• Gives you access to meaningful reports to help you better manage your patients/practice
• Allows access to payer networks that might otherwise be closed to enrollment
• Provides clinicians a higher level of collaboration, sharing of best practices, and clinical satisfaction
• Enables contracting with payers through a single signature authority
• Provides a mechanism for providers to participate in savings when performance benchmarks are achieved

If you are either (1) Employed by the Mount Sinai Health System and have a private practice and want to join through your private practice; or (2)  In private practice and are credentialed through the Mount Sinai Health System with voluntary privileges, you can apply by filling out the “Contact MSHP” form below. We will need to verify your credentials.  Please also fill out the provider application form below. Please be sure to include your CAQH ID on the form and email it to us at MSHP@mountsinai.org.

If you are not credentialed by The Mount Sinai Health System, you can apply by:

• Providing us your CAQH ID using the “Contact MSHP” form below. If you do not have a CAQH, please log in to https://proview.caqh.org/Login and create a CAQH application, which will generate a CAQH ID; and
• Filling out the provider application form below and emailing it to us at MSHP@mountsinai.org

Note: The CAQH Universal Credentialing DataSource is designed to make the credentialing process easier for providers by gathering data in a single repository that may be accessed by participating health plans and other healthcare organizations.
